Extra Programing Sign-up

Please use these forms to select the options you plan to use on a regular basis. “Regular attendance” includes any consistent schedule. Whether it's one day a week or more, and should last for at least one month. Missed Day are not transferable.

If you prefer to use a program on a drop-in basis, simply notify the office in advance or as soon as possible with the specific days and programs you'd like to add. This can be through E-mail or Brightwheel. 

Your chosen schedule will remain in place unless you notify us of a change. To modify or end your registration, please contact Katie Schroeder at kschroeder@stdavidsdayschool.net. You may make adjustments at any time. 

Monthly billing will occur at the beginning of each month. Charges for drop-in services will be billed as they occur.

Regular discounted attendance of Early Morning Drop-Off,  Lunch Bunch and After School will be billed monthly.

Please complete one registration per child.
